Job summary

Union Health is seeking a pharmacist for the Terre Haute campus, focused on outpatient dispensing in UAP Clinic Pharmacy and Lobby Pharmacy. You will collaborate with nurses and providers to ensure accurate, timely medication access for patients being treated in the health system.

The role requires a Pharm.D., Indiana licensure eligibility, and ASHP residency experience is preferred. A $50,000 sign-on bonus accompanies competitive benefits and professional development opportunities.

Qualifications

• Advanced Pharmacy Degree (Pharm.D.)
• Board Certification preferred in the first year of eligibility.
• One to two years’ experience in ASHP accredited residency or equivalent experience preferred.
• Licensed or eligible for licensure in the State of Indiana; licensure within 75 days after employment start.

Responsibilities

• Performs drug information functions for patients and staff using Lexicomp and internet resources.
• Conducts drug dosing consultations and participates in PI activities for dosing services.
• Serves on committees such as P&T, Stroke, Infection Control, Medication Safety and Readmissions.
• Engages in pharmacy education activities for medical staff, nursing, and ancillary departments.
• Contributes to drug formulary management and professional development.
• Adheres to health system policies and state/national pharmacy laws.

The Pharmacy Department at Union Health is looking for team members for the Terre Haute campus. We are looking for individuals who are compassionate, fully committed to providing the highest quality health care experience and dedicated to supporting their team who work in a fast-paced environment.

Pharmacists practicing at UAP Clinic Pharmacy and Union Hospital Lobby Pharmacy work primarily in an outpatient dispensing role. UAP Clinic Pharmacy, located in Union’s Medical Office Building, is a full retail pharmacy that serves patients of the health system, patients of the community and is the preferred pharmacy for all Union Health employees. UAP Clinic Pharmacy also distributes medications to all of Union Health’s outpatient Primary Care and Specialty offices. Union Hospital Lobby Pharmacy is located in Union's Main Hospital Building and serves patients being discharged from the hospital, emergency department and ambulatory surgery. Lobby Pharmacy staff work closely with nurses, providers and the Transitions of Care team to provide medications to patients in-hand upon discharge from the health system.

• Performs drug information functions for patients, medical and ancillary departments, and pharmacy staff, utilizing Internet search engines and Lexicomp effectively; assists in training staff.
• Conducts drug dosing consultations, demonstrating personal knowledge through consults and participating in PI activities for drug dosing services.
• Serves on various committees assigned by pharmacy administration, such as P & T, Stroke, Infection Control, Medication Safety, Glucose Control, Pain Team, Sepsis Team, Antimicrobial Stewardship Team, Policy Committee, Readmissions.
• Engages in pharmacy education activities, including planning staff development programs, providing educational programs to medical staff, nursing, and ancillary departments, and offering patient-oriented education.
• Involvement in clinical PI/documentation activities, assisting in drug formulary management processes.
• Focuses on professional development and competency assurance, attending educational offerings, observing health system policies, complying with state and national pharmacy laws, performing special projects, and exhibiting dependability.

Education, Training & Experience:

• Advanced Pharmacy Degree (Pharm.D.)
• Board Certification preferred in the first year of eligibility.
• One to two years’ experience in ASHP accredited residency or equivalent experience preferred.
• Licensed or eligible for licensure in the State of Indiana. Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position. Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.
