Job summary

Union Health in Terre Haute, IN is seeking an Inpatient Clinical Pharmacist to optimize patient outcomes by managing medication use across hospital settings. You will ensure services meet professional standards and organizational policies while supporting a compassionate care culture.

Qualifications

• Advanced Pharmacy Degree (Pharm.D.)
• Board Certification preferred in the first year of eligibility
• One to two years experience in ASHP accredited residency or equivalent experience preferred
• Licensed or eligible for licensure in the State of Indiana; licensure within 75 days after employment start
• Good verbal and written communication skills
• Continuing education per state licensing requirements
• Flexible work schedule to meet department needs
• Excellent grasp of standard written and verbal English

Clinical Pharmacist Essential Job Duties:

• Performs drug information functions as necessary to meet the needs of the patients, medical, nursing, ancillary departments, and pharmacy staff.
• Observes and complies with health system policies and regulations.
• Observes and complies with state and national pharmacy law
• Performs special projects at request of pharmacy administration
• Performs all other duties as assigned by pharmacy administration.
• Verifies medication orders
• Adjust medication orders per hospital approved policies (i.e. renal dose protocol, therapeutic interchanges, IV to PO conversion, etc.)
• Verifies/checks IVPBs, LVP, PN, oral dosage forms, topicals, vials, and other injectables

Education, Training & Experience:

• Advanced Pharmacy Degree (Pharm.D.)
• Board Certification preferred in the first year of eligibility.
• One to two years experience in ASHP accredited residency or equivalent experience preferred.
• Licensed or eligible for licensure in the State of Indiana. Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position. Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.
