Job summary

Mountain Park Health Center is seeking Pharmacists to join our established team delivering high quality pharmaceutical care at Baseline and Maryvale clinic sites. You will review medication orders, dispense medications, counsel patients and staff, and collaborate with physicians to ensure safe, effective therapy.

Responsibilities include ensuring compliance with laws and policies, conducting drug utilization reviews, maintaining accurate records in pharmacy software and EHR, and staying current

Qualifications

• Review and evaluate medication orders for appropriateness and completeness with patient data.
• Provide patient counseling on prescription and non-prescription medications and collaborate with providers as needed.
• Maintain accurate medication records in pharmacy software and EHR.
• Ensure reporting for adverse drug reactions and medication programs.

Responsibilities

• Review medication orders and evaluate appropriateness and completeness with respect to patient data.
• Complete prescription orders: data entry, verification, labeling, final product verification, and staging for delivery.
• Utilize clinical pharmacy skills to assess dosage, contraindications, and drug interactions.
• Monitor 340B guidance and Medicaid changes to ensure regulatory compliance.
• Travel to multiple clinic locations as assigned and maintain regular attendance.
