Job summary

Overview

Schedule: Part time varied, 8 - 10 hour shifts, 1 night weekly on call. Weekends/Holidays occasional as needed.

How you\'ll contribute
  • Pharmacist who excels in this role: Coordinate and provide pharmaceutical care for patients, collaborating with interprofessional team to optimize safe, effective medication use.
  • Review and verify orders, manage drug therapy protocols, educate staff/patients, and support quality and regulatory standards.
  • Manage drug therapy protocols as part of an interdisciplinary team.
  • Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
  • Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
  • Document interventions and outcomes for drug therapy quality improvement.
  • Verify medication orders for accuracy/completeness prior to entry.
  • Check filled medications prepared by technicians.
  • Maintain proficiency with pharmacy automated systems and software.
  • Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
  • Participates in performance improvement and required continuing education.
