Job summary

Canyon Vista Medical Center in Sierra Vista is seeking a Pharmacist to join Lifepoint Health. The role emphasizes coordinating pharmaceutical care with an interprofessional team to optimize safe, effective medication use, review and verify orders, and support quality and regulatory standards.

Responsibilities

• Coordinate and provide pharmaceutical care for patients with interprofessional team to optimize safe, effective medication use.
• Review and verify orders, manage drug therapy protocols, educate staff/patients, and support quality and regulatory standards.
• Manage drug therapy protocols as part of an interdisciplinary team.
• Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
• Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Document interventions and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy/completeness prior to entry.
• Check filled medications prepared by technicians.
• Maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participates in performance improvement and required continuing education.
