Job summary

PharMerica, an affiliate of BrightSpring Health Services, is hiring a full-time Pharmacist in Huntington, WV for our long-term care focused closed-door pharmacy team.

You will evaluate orders, provide clinical recommendations, maintain resident profiles, ensure monitoring labs, collaborate with physicians and nurses, and uphold regulatory compliance. Benefits and growth opportunities are available.

Qualifications

• Bachelor’s degree in Pharmacy or PharmD preferred.
• One year experience in long-term care desired.
• Current pharmacist license in the state of practice.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration.
• Provide clinical recommendations to physicians and healthcare staff regarding therapy changes.
• Maintain patient/resident profiles with medical history, labs, and medications.
• Ensure lab monitoring for high-risk meds is ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Document all clinical communications and interventions in medical records.
• Implement policies for safe handling, dispensing, and medication management.
• Ensure compliance with state and federal regulatory requirements.
• Report medication-related errors and adverse events to Pharmacy Manager.
• Educate clinical team on safe medication handling procedures.
