• Current registered pharmacist license in Florida is required.

Responsibilities

• Review and monitor patient medication therapy and orders for safety and efficacy; advise care team on therapy and document interventions.
• Order verification and medication profile reviews; ensure compliance with laws and regulations.
• Provide leadership and participate in problem solving and evaluation.

Job Shift: Rotate- M-F 9:00 am - 5:30 pm. Every third weekend

Job Type: Full Time- M-F 9:00 am - 5:30 pm. Every third weekend

Up to $10,000 SOB for qualified candidates, paid over 24 months

The Staff Pharmacist provides pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Additionally: ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization.

Responsibilities

• Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.

Qualifications

Education

• Required: Pharm. D. or BS in Pharmacy
• PGY-1 residency or at least 3 years of equivalent experience with BCPS

Experience

• Preferred: Hospital experience

License

• Required: Current registered pharmacist license in the State of Florida

Certifications

• Required: BLS within 30 days of hire (ACLS and/or PALS if code attendance required)
• Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
