Job summary

St. Mary’s Medical Center Hospital in West Palm Beach, FL, seeks a Staff Pharmacist to provide pharmaceutical care, oversight, and dispensing per physician prescriptions while ensuring regulatory compliance. You will deliver drug information and participate in medication safety initiatives within a hospital setting.

The role requires a Pharm.D. or a BS in Pharmacy, a current Florida pharmacist license, and BLS within 30 days of hire; hospital experience is preferred.

Qualifications

• Required: Pharm. D. or BS in Pharmacy.
• Current Florida pharmacist license.
• BLS within 30 days of hire (ACLS/PALS if code attendance required).
• Preferred: Hospital experience.
• Board Certification in Pharmacotherapy (BCPS), CGP, etc.

Responsibilities

• Review and monitor patient medication therapy and orders for safety and efficacy; advise care team and document interventions.
• Order verification and medication profile reviews for duplications, interactions, allergies, dosing and frequency.
• Prepare, label, and dispense medications per physician prescriptions; ensure compliance with laws and regulations.
• Demonstrate leadership, problem solving, intervention, and evaluation within the pharmacy team.

Up to $10,000 SOB for qualified candidates, paid over 24 months.

The Staff Pharmacist provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Additionally: ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization.

Responsibilities

• Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient's medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.
