Job description

Overview

Engages patients and healthcare professionals to efficiently and effectively maximize therapeutic outcomes. The pharmacist is responsible for tasks such as dispensing medications, monitoring drug therapies, working under collaborative practice agreements or protocols, and providing consultation and education to patients and staff.

Position type: Full‑time, 40 hours per week. Overnight shift schedule: 7‑on, 7‑off.

Responsibilities

• Gather a complete medication history and reconcile medication lists.
• Complete comprehensive medication reviews to make evidence‑based recommendations ensuring medications are necessary, safe, and cost‑effective.
• Identify and record suspected adverse drug reactions, adherence gaps, and barriers to patient adherence.
• When not practicing under a collaborative practice agreement, contact providers regarding any unsafe or inappropriate medication use.
• Accurately document all clinical activities and interventions in the electronic record.
• Serve as a consultation resource to medical professionals and answer inquiries on drug therapy and products.
• Consult patients and caretakers on all aspects of medication therapy when necessary.
• Assist in discharge planning and counsel patients and caretakers on proper drug administration, potential side effects, interactions, and precautions.
• Contribute to development, maintenance, and management of medication‑related guidelines defined by protocol or collaborative agreements.
• Detect and report medication errors and near misses.
• Ensure readiness for regulatory and accreditation surveys.
• Participate in quality improvement and medication use review activities of the department.
• Prepare and deliver presentations to facilitate learning or disseminate evidence‑based best practice to pharmacists, nurses, physicians, and other healthcare professionals.
• Utilize a solution‑based approach to ensure heightened patient satisfaction and care.
• Anticipate patient needs and proactively identify solutions.
• Communicate effectively between departments and teams to ensure optimal patient outcomes.
• Actively participate in driving programs and initiatives to improve patient quality of care, medication compliance, and overall efficiency.
• Pursue and maintain certifications necessary to remain compliant with accrediting bodies and standards.
• Participate in the ordering and receiving of controlled substances and other medications as appropriate.
• Effectively utilize and supervise technicians to maintain an accurate and efficient pharmacy; redirect staff when necessary to achieve service performance goals.
• Use professional judgment to resolve complaints.
• Act as pharmacist‑in‑charge in the absence of pharmacy managers and supervisors.
• Oversee day‑to‑day operation of the pharmacy, including staff management, compliance with state and federal guidelines, and medication inventory.
• Perform all registered pharmacist duties, including providing vaccinations approved by the NH Board of Pharmacy.
• Be prepared for potential occupational exposure to blood or bodily fluids; complete required training and receive hepatitis B vaccinations.
• Perform other duties as required or assigned.

Qualifications

• Graduate of an ACPE‑accredited School of Pharmacy with a Bachelor of Science in Pharmacy.
• Doctor of Pharmacy (Pharm D) degree preferred.
• Post‑graduate residency and/or board certification preferred.
• Ability to work independently with minimal supervision, set priorities, make critical decisions, and respond quickly to emergency requests.
• Strong customer‑service focus and excellent working relationships.
• Strong written and verbal communication skills.
• Must be currently licensed to practice in the state of NH and willing to seek license reciprocity in multiple states if required.

Required Licensure and Certifications

• Current, unrestricted pharmacist license to practice in the state of NH.
• Board Certification in Specialty Pharmacy (CSP) within 3 years of hire for clinical pharmacists in specialty pharmacy (unless part‑time or per‑diem).
• BLS Certification within 30 days of hire where required for immunization administration.
