Job Title: Pharmacist, Emergency

Employment Type: Part time, Per Diem

Shift: Rotating shifts, weekends, holidays as needed per department needs

Location: Mercy Fitzgerald Hospital - Darby, PA

Position Purpose

We are hiring pharmacists to support operations and our expanded emergency pharmacy program. We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.

As a Pharmacist, you will:

• Appropriately assess patient data to identify medication-related problems through profile reviews, pharmacy consults, and interdisciplinary rounding.
• Participate in medical emergencies and other formal alerts such as trauma, stroke, sepsis, and STEMI as required.
• Review and interpret providers' orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Check for drug incompatibilities and allergies, and take appropriate action when necessary. Update medication history for all admitted patients.
• As necessary, consult with providers, nurses, and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
• Provide accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
• Document patient care activities including therapy plan, monitoring, and follow-up in the electronic medical record as per departmental guidelines.

Minimum Qualifications

• Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
• Registered pharmacist in Pennsylvania required.
• Preferred: Experience with Pyxis MedStation or EPIC.
• BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire).
