Job summary

Mercy Fitzgerald Hospital in Darby, PA (part-time, per diem) is seeking an Emergency Pharmacist to support an expanded hospital program. The role includes direct patient interaction for histories and counseling, and participation in emergencies within the hospital.

Responsibilities include reviewing orders, coordinating with care teams, and documenting in the EMR. Requires Pharm.D., PA license, and hospital experience with ACLS/BLS. Prior Pyxis/EPIC experience is preferred.

Qualifications

Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
Registered pharmacist in Pennsylvania required.
Preferred: Experience with Pyxis MedStation, EPIC
BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)

Responsibilities

Assess patient data to identify medication-related problems via profile reviews and rounding.
Participate in medical emergencies and alerts such as trauma, stroke, sepsis and STEMI as required.
Review providers' orders to ensure appropriate drug, form, and dose for age/weight/comorbidities.
Consult with providers/nurses on pharmacotherapy optimization to improve patient outcomes.
Provide accurate drug information and policy guidance to staff and patients.
Document care activities including therapy plan, monitoring, and follow-up in EMR.
