Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification may be required based on area of practice
• ACPE-accredited pharmacy degree or NABP FPGEC certificate typically required

Responsabilités

• Oversees pharmaceutical care by retrieving literature and designing therapy regimens
• Provides counseling to patients and caregivers on medication therapy, adverse effects, compliance, and administration
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens and implements recommendations
• Reviews and interprets medication orders and prescriptions
• Supports medication safety, quality improvement, and formulary management
• Prepares and dispenses medications following state and federal regulations
• Mentors pharmacy students, residents, and learners as assigned
