• Licensed in state of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification per practice area.
• Credentialing and privileging as applicable.

Responsibilities

• Oversees pharmaceutical care and designs therapy regimens.
• Counsels patients and caregivers about medications and adherence.
• Interprets medication orders and prescriptions accurately.
• Supports medication safety and quality improvement activities.
• Collaborates to manage formulary adherence and access to non-formulary meds.
• Precepts students and mentors pharmacy teammates.
• Ensures compliance with policies and regulations.

Education

Bachelor’s or Doctorate degree in pharmacy
