Clinical Pharmacist - Home & Specialty Infusion Services

Location: Oklahoma City, OK

Duration: 25-Week Contract

Schedule: Full-Time, 40 Hours/Week

Position Overview

A leading specialty pharmacy organization is seeking an experienced Clinical Pharmacist to support patients receiving infusion therapies in the home and ambulatory setting. This role combines clinical pharmacy practice, medication management, sterile compounding oversight, and interdisciplinary collaboration to ensure safe and effective patient care. The pharmacist will work closely with physicians, nurses, and pharmacy staff to review patient therapies, monitor clinical outcomes, and provide expert guidance on complex medication regimens.

Schedule

Rotating Weekday Schedule (Monday-Friday):

• 8:00 AM - 5:00 PM
• 8:30 AM - 5:30 PM
• 9:00 AM - 6:00 PM

On-Call Requirement

• Approximately one weekend per month
• Friday 5:30 PM through Monday 8:30 AM

Key Responsibilities

• Review, verify, and process medication orders for infusion patients.
• Monitor drug therapies, laboratory values, and patient outcomes to ensure safe and effective treatment.
• Collaborate with physicians, nurses, and healthcare professionals regarding medication management and therapeutic recommendations.
• Conduct clinical assessments and identify potential drug-related concerns.
• Provide medication consultation and education to providers, patients, and caregivers.
• Oversee pharmacy technician activities and workflow as permitted by state regulations.
• Verify medications, supplies, and equipment before dispensing.
• Support quality assurance initiatives and maintain compliance with regulatory standards.
• Document clinical interventions and patient care activities.
• Participate in sterile compounding operations and supervision of clean room practices.

Ideal Background

• Active Pharmacist licensure
