PharMerica in Greensburg, PA, is hiring a full-time Pharmacist for the 2nd shift in a closed-door long-term care pharmacy. You will work evenings in a non-retail setting, ensuring safe medication use for residents and collaborating with a multidisciplinary team.

You will evaluate orders, provide clinical recommendations, maintain resident profiles, monitor high-risk meds, and support staff with safe medication handling. A BS in Pharmacy or PharmD and valid PA license are required.

Qualifications

• Bachelor’s degree in Pharmacy required or PharmD preferred.
• One year of long-term care experience desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS, DEA, FDA guidelines and safety practices.
• Proficiency with EMR and pharmacy information systems.

Responsabilités

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain up-to-date patient/resident profiles and medication records.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians and nurses to support clinical care.
