Job summary

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD) in California is seeking a Pharmacist to verify orders, dispense medications, and supervise technicians within an on-site pharmacy. You will collaborate with the clinical team, counsel patients, and participate in multidisciplinary care for hemophilia and related blood disorders.

The role requires an ACPE-accredited PharmD, a California license, and at least four years in outpatient settings, with strong Excel/EPIC

Responsibilities

Job description

Job Summary

The Pharmacist will verify orders, dispense medications, ensure pharmacy programs and delivery services are in accordance with CIBD standards and applicable State and Federal regulations, and consult with the prescriber and patient. The Pharmacist will supervise pharmacy technician activities, precept students, and perform other duties as assigned.

Schedule: M - F, 8:30 - 5:00

Location: On-site

Essential Duties

• Oversee all pharmacy dispensing activities, including providing drug information; reviewing for drug interactions, adverse drug events, and allergies; and monitoring medication adherence and therapeutic outcomes to ensure safe and effective medication use.
• Collaborate with the clinical team to identify and resolve patient care issues.
• Provide accurate drug information to healthcare providers and counsel patients and caregivers on proper medication use.
• Enter and verify orders prior to release in Pyxis, ensuring accuracy, appropriateness and patient safety.
• Participate in the annual comprehensive clinic visits for patients with hemophilia as part of multidisciplinary care delivery.
• Perform routine daily and monthly cleaning and disinfection of the IV room in compliance with USP <797> and established policies and procedures.
• Serve as a preceptor to pharmacy students during clinical rotation.

Requirements

• Graduate from an ACPE-accredited School of Pharmacy with a Pharmacy Degree (PharmD).
• Minimum four (4) years of experience in outpatient/ambulatory pharmacy experience.
• Experience working in a variety of healthcare settings, including hospital, specialty clinic or infusion center.

Required Licenses/Certifications

• Active California State Board of Pharmacy pharmacist license in good standing.
• Basic Life Support (BLS) Certification.
• HIPAA Training (or ability to complete within one month of hire).

Additional Required Qualifications

• High proficiency in the use of Microsoft Excel, Word, and Outlook.
• Demonstrated effective interpersonal, verbal, and written communication skills.
• Experience leading the development and implementation of processes and initiatives to support growth and integration of specialty pharmacy services within the clinic.
• Comprehensive knowledge of applicable state and federal pharmacy laws governing daily pharmacy operations.
• Strong attention to detail and accuracy in the completion of all assigned tasks.
• Effective time management and organizational skills.
• Ability to adapt to evolving priorities and implement short-term objectives in a dynamic work environment.
• Ability to make drug-related recommendations and clinical interventions based on sound clinical judgement.

Preferred Qualifications

• Completion of an ASHP accredited residency program.
• Knowledge of 340B program administration.
• Experience managing care for patients with hemophilia, including gene therapy, and other blood disorders.
• Proficiency with Pyxis and electronic health record (EHR) systems such as Epic.
• Strong knowledge of sterile compounding and regulatory requirements and standards governing infusion pharmacy practice, including USP <797> and USP <800>.
• Bilingual – Spanish.

Compensation listed is the expected salary base at time of hire. Actual compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience.

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non-profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with Hemophilia, Sickle-Cell Anemia, Thalassemia, Von Willebrand Disease, Mast Cell Activation Syndrome (MCAS) and other related benign hematology conditions. CIBD is the regional grantee for the Western States Region for Hemophilia, the Pacific Sickle Cell Regional Collaborative (PSCRC) and Networking California for Sickle Cell Care (NCSCC) providing regional leadership, fiscal and administrative oversight, grants management, capacity building, clinical education, technical assistance, evaluation, and outreach to public and private health agencies at the local, state, and federal levels.
