Job summary

Country Meadows Retirement Communities in Hershey, PA is seeking a Personal Care Coordinator who will oversee shifts, maintain schedules, and implement individualized service plans to ensure resident satisfaction. You will supervise and mentor personal care associates, coordinate care with program managers, and assess changing resident needs to update care plans.

Pay starts at $21-23/hour, with potential to earn more based on experience; full-time hours 3 pm–11 pm; extensive benefits include

Qualifications

• Prior personal care or health care experience as a PCA.
• Supervisory experience required.
• High School Diploma or GED.
• Current CPR certification or willingness to obtain.

Responsibilities

• Maintains shift assignments, shower schedules, and room cleaning schedules as needed for all shift assignments updating point of care, printing and reviewing task reports.
• Upholds open and effective communication with the Program Managers and the PCA's to provide consistent quality care for residents.
• Assists the Program Managers in performing ongoing assessments of existing residents to determine their changing need and provides input for the service plan.

In the role of Personal Care Coordinator, you will encourage, support and implement a service plan for each resident to ensure the most appropriate care and satisfaction is obtained. You will also supervise, guide, and mentor the personal care associates to make sure life gets better for our residents.

Full time 3 pm to 11 pm 75 hours in a 2 week pay period
