Overview

CNA/Personal Care Attendant

PACE (Program of All-Inclusive Care for the Elderly)

Monday-Friday, 8:00AM-4:30PM, rotating weekends

Under the supervision of the Clinical Manager PACE, provides personal care and services according to individual care plans for assigned participants of Siouxland PACE. Adheres to the principles of Service Care Standards and the philosophy, mission and values of the Siouxland PACE program in all aspects of job performance.

Responsibilities
  • Maintain knowledge of applicable laws, regulations, policies, and procedures to ensure compliance and ethical behavior.
  • Accurately document all participant care activities in accordance with Siouxland PACE policies.
  • Follow safety and infection control policies and respect participants' rights and confidentiality.
  • Work effectively with individuals from diverse backgrounds.
  • Participate in interdisciplinary team reporting and planning.
  • Assist with activities of daily living, including toileting, bathing, grooming, and eating assistance.
  • Support physical and recreational activities, and update care plans accordingly.
  • Monitor for changes in condition and report promptly.
  • Provide emotional support and reminders for nursing instructions and medication adherence.
  • Collaborate with departments to meet participant needs and maintain a safe environment.
  • Assist with transportation, laundry, housekeeping, and recreation activities as needed.
  • Attend to personal care areas and assist with ambulation and transfers.
Qualifications
  • High School Diploma, GED, or relevant work experience.
  • Completion of at least 75 hours of nursing assistant training.
  • Current CPR and Mandatory Reporter certifications.
  • 12 hours of related in-services annually.
  • Valid driver’s license and auto insurance for travel to participant homes.
Skills and Abilities
  • Follow directions accurately and independently.
  • Knowledge of elderly care needs and dementia techniques.
  • Proper use of assistive devices and safety equipment.
  • Problem-solving, communication, and computer skills.
  • Dependable, flexible, and self-directed with the ability to work independently and as part of a team.
  • Willingness to travel occasionally to participant homes.
