Job Description - Health Maintenance Attendant

Reports To: Director of Clinical Services, Director of Operations, Supervisor

Supervises: NONE

Duties and Responsibilities
  • Personal Care Assists the client with or provides personal care as assigned by the plan of care, including bathing, care of hair, skin, and mouth, toileting, dressing, transferring, and ambulation.
  • Assists with mobility, positioning, and transfers in a safe manner that protects both the client and the employee.
  • Recognizes and responds appropriately to changes in the client's condition, reporting concerns promptly to the supervisor or manager.
  • Service Delivery & Scheduling Provides services when scheduled and communicates promptly to the Agency and client regarding any schedule changes. Accomplishes job responsibilities in a timely, safe, and professional manner. Participates in client case conferences according to Agency policy and procedures.
  • Documentation & Communication Documents activities performed in the client's home at the time of the visit. Documents client concerns and difficulties and reports the same to the supervisor or manager. Completes and submits all required paperwork accurately and on time.
  • Compliance & Professional Standards Represents the Agency in a professional manner and dresses appropriately for job duties. Maintains client confidentiality in accordance with HIPAA and Agency policy. Adheres to all company policies and procedures outlined in the Employee Handbook, Employee Agreement, or as communicated by the executive team. Performs other duties as assigned.
Requirements
  • Required Ability to read and interpret documents such as safety rules, care plans, and procedure manuals
  • Required Ability to communicate effectively, both verbally and in writing, with clients, families, and Agency staff
  • Required Ability to maintain a high level of confidentiality in all aspects of job responsibilities
  • Required Ability to solve practical problems and improvise in an unstructured home setting
  • Required Ability to establish and maintain effective working relationships with fellow employees, clients, and supervisors
  • Required Ability to frequently lift and/or move up to 50 pounds and to perform physical tasks including sitting, reaching, stooping, kneeling, and crouching
  • Required Possession of a valid Colorado Driver’s License with applicable car insurance, or reliable transportation to all accepted assignments
  • Preferred High school diploma preferred, GED accepted
  • Verifiable work experience as a Personal Care Attendant or Personal Care Provider
  • Prior experience in a home health care or IHSS setting
  • Familiarity with Medicaid programs and IHSS services
