Job summary

Black Hills Special Services Cooperative (BHSSC) is seeking a Support Coordinator to serve as the primary contact for individuals receiving developmental disability services in Hot Springs, SD. The role collaborates with participants, families, legal representatives, staff, and Conflict Free Case Managers to ensure person-centered services and timely documentation.

Qualifications

• Bachelor's degree (or equivalent) in human services or related field.
• Strong verbal and written communication skills.
• Knowledge of person-centered services, rights, and regulatory requirements.
• Ability to pass a background check.
• Ability to pass a physical & drug screening; lift up to 50 lbs.

Responsibilities

• Manage an assigned caseload and be the primary BHSSC contact for individuals receiving developmental disability services.
• Coordinate person-centered planning and supports with individuals, families, legal representatives, and other teams.
• Monitor ISPs to ensure goals, services, and supports are delivered and updated as needed.
• Maintain HIPAA-compliant documentation and records across Therap and other systems.
