Job summary

Mosaic Beatrice seeks a Habilitative Coordinator / Case Manager to empower individuals and coordinate services in the Beatrice community. This full-time, salaried role blends advocacy with program development to promote independence and growth.

You’ll oversee a caseload of up to 22 individuals, lead annual meetings with CFA/POM/SIS assessments, collaborate with families and supervisors to ensure admissions and compliance, and train staff in behavior modification while maintaining clear

Qualifications

  • Bachelor’s Degree in Psychology, Special Education, or a closely related field.
  • At least 3 years of hands-on experience with individuals with intellectual/developmental disabilities.
  • Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.
  • QIDP / QMRP certification (or state eligibility), Med Aide 40, CPR/First Aid and Mandt certification required (or to obtain after hire).
  • Exceptional critical thinking, problem-solving, written communication, and time-management skills.

Responsibilities

  • Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs.
  • Coordinate and present CFA, POM, SIS assessments, lead annual review meetings, and present before the Human Rights Committee.
  • Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to drive admission dates and maintain regulatory compliance.
  • Train direct care staff in behavior modification techniques and help develop innovative service methodologies.
  • Monitor the quality of care and maintain clear, professional documentation.

What You’ll Do

As a key leader in service delivery, you’ll blend hands-on advocacy with strategic coordination:

  • Empower & Advocate:Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs that support personal growth and independence.
  • Lead Annual Meetings & Assessments:Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.

Requirements
What You Bring to the Team

Education:REQUIRED -Bachelor’s Degree in Psychology, Special Education, or a closely related field

Experience:At least 3 years of hands-on experience working with individuals with intellectual/developmental disabilities.
