Qualifications

• Experience with housing applications and homeless services.
• Experience with individuals with mental illness, substance use, or co-occurring disorders.
• Experience in safe haven, outreach, or supportive housing.

Responsibilities

• Work with interdisciplinary team to promote wellness and secure housing.
• Conduct housing eligibility assessments for clients.
• Meet with clients to set goals and monitor progress.
• Prepare housing applications and interview coaching.
• Develop client workshops and individual sessions for housing readiness.
• Navigate barriers to achieve permanent housing.
• Track placements and maintain client records.
• Engage with landlords and providers to ensure stability.
