Qualifications

  • Certification in First Aid, CPR.

  • Must possess Valid NYS Driver’s License.

Responsibilities

  • Meet families within 48 hours of arrival to assess housing needs.
  • Prepare Initial Housing Plan and progress notes in the Uniform Client Chart.
  • Coordinate with landlords and real estate brokers to secure permanent housing.
  • Advocate for subsidies and housing programs for clients.
  • Document referrals and maintain client charts for ongoing housing support.
  • Assist clients with completing relevant subsidies and housing program applications.
