Job summary

WestCare is seeking a Permanency Specialist to secure safe and timely permanency outcomes for children in foster care. You will track permanency data, identify barriers, and work with case managers and supervisors to implement strategies for successful permanency.

You will facilitate staffing, coordinate follow-up activities, and engage families in reunification and permanency planning while maintaining accurate records and ensuring compliance with policies and court directives.

Qualifications

  • Bachelor’s degree in social work or a related field with at least two years of human services experience.
  • Two years of supervised experience in child welfare.
  • Certified Case Manager (CBHCM or CWCM) preferred.
  • Bilingual abilities preferred.

Responsibilities

  • Prioritize permanency efforts and monitor children likely to exit care.
  • Create monthly permanency data reports and analyze barriers.
  • Maintain accurate permanency records in the system and coordinate staffings.
  • Schedule and facilitate permanency staffing and track follow-up tasks.
  • Engage families in permanency and reunification planning.
  • Ensure compliance with policies and court directives.
  • Attend court as necessary.
  • Collaborate with case managers, supervisors, and external partners to remove barriers.
