Responsibilities

  • Evaluate residents across skilled nursing, rehabilitation, memory support, and assisted living populations.
  • Assess speech, language, cognition, communication, and swallowing needs.
  • Develop individualized treatment plans based on resident goals and clinical presentation.
  • Provide evidence-based speech-language therapy interventions.
  • Support residents with cognitive-communication needs, including those experiencing cognitive decline or dementia.
  • Evaluate and treat swallowing and feeding disorders when indicated.
  • Collaborate with nursing, dietary, therapy, and care teams to promote safe and effective care.
  • Provide education to residents, families, caregivers, and staff.
  • Participate in interdisciplinary care planning.
  • Support functional communication and independence in daily activities.
  • Monitor progress and modify treatment plans as needed.
  • Complete required clinical documentation accurately and efficiently.
  • Contribute to a positive environment focused on resident dignity, engagement, and quality of life.

Compensation: $39.50–$44.00 per hour
Schedule: 40 hours per week | Monday–Friday
Weekend Rotation: Yes
Setting: Skilled Nursing & Assisted/Senior Living Community
Facility Size: 80+ Skilled Nursing Beds + 10 Assisted Living Residents
Employment Type: Permanent, Full-Time

Overview

The community provides post-hospital and short-term rehabilitation, long-term care, respite services, specialized memory support for individuals living with Alzheimer’s disease or dementia, and assisted/senior living services.
