Responsibilities

• Assess patient clinical and psychosocial needs and develop, implement, and evaluate a plan of care.
• Coordinate with the interdisciplinary team to ensure safe patient transitions and evidence-based care.
• Delegate and supervise non-professional staff to maintain quality patient care.
• Complete clinical and quality documentation per RHS standards and regulatory requirements.
• Respond to emergencies and allocate resources to meet patient needs.
