Nemours IMFH is expanding social work services within the Fetal Care Center to provide comprehensive perinatal psychosocial care from prenatal through postpartum. You will perform psychosocial assessments, crisis intervention, and SBIRT-based screening, linking families to community resources and supports in a multidisciplinary team.

Qualified candidates hold a Master’s in a Human Services field with LMSW eligibility; PMH-C preferred and Delaware licensure favored.

Qualifications

• Master’s degree in a Human Services related field with at least 2 years post-master’s work experience.
• Active Perinatal Mental Health Certification (PMH-C) preferred.
• Active Delaware LCSW preferred.

Responsabilités

• Conduct perinatal psychosocial assessments and support coping, safety, and decision-making.
• Use SBIRT model for universal perinatal mental health screening and brief interventions with referrals.
• Connect families with community resources to address SDOH needs.
• Provide crisis intervention and protective services, including abuse assessments.
• Coordinate with Perinatal Bereavement Coordinator on disposition planning and resources.
• Support discharge planning and care coordination for timely, safe outcomes.
• Participate in interdisciplinary planning, rounds, trainings, and quality initiatives.
• Maintain compliant documentation in medical and research records.
• Respond to communications per hospital policies; may lead group education.
• May train/ supervise social work students and new staff.

Formation

Master's degree in a Human Services related field

Active Delaware LMSW license (minimum)
