Job summary

Sunrise Hospital and Medical Center in Las Vegas is seeking a Perinatal Nurse Navigator to join a multidisciplinary team. You will advocate for patients, coordinate plans of care, and guide families through the healthcare system to improve care and outcomes.

This on-site role supports MountainView Hospital and Southern Hills Hospital, with a Monday–Friday 8:00 AM–5:00 PM schedule. RN experience with patient education and navigation is preferred; BLS is required.

Qualifications

• 1-3 years of experience in Nursing – Required
• 3+ years of experience as a Nurse Navigator and/or patient educator in service line – Preferred
• Associate’s Degree in Nursing required
• Bachelor’s Degree in Nursing preferred

Responsibilities

• Serve as a patient advocate
• Supports patients through complex healthcare environments to optimize care
• Coordinate plan of care and appointments; assist in access to care
• Assist with transitions between care modalities and facilities
• Attend Multidisciplinary Meetings and document data
