Job summary

Native American Health Center in Oakland, CA seeks a Comprehensive Perinatal Health Worker to ensure members access high-quality care, coordinate services, and manage referrals within CPSP guidelines.

You will handle scheduling, medical records, ICPs, and collaboration with care teams, while maintaining HIPAA compliance and using EHR systems. Bilingual in English and Spanish preferred; day shift.

Qualifications

• Minimum 12 months experience in a perinatal program or a health education-related bachelor’s degree.
• Fluency in English and Spanish may be required.
• Vaccination and safety requirements must be met prior to start date.
• Proficient with standard office software and healthcare documentation systems.
• Strong customer service and ability to multi-task in a fast-paced environment.

Responsibilities

• Provide new member orientations for perinatal members and CPSP assessments.
• Coordinate care with providers, medical assistants, and care coordinators.
• Process perinatal referrals and schedule external appointments.
• Track referrals, schedule ultrasound and tests, and ensure attendance.
• Maintain Individualized Care Plans and HIPAA-compliant documentation.
