Job summary

Native American Health Center seeks a Comprehensive Perinatal Health Worker to ensure members access high quality care within CPSP. You will coordinate with the care team, process referrals, schedule external appointments, and monitor ICPs while addressing barriers to access.

Requirements include a health education degree, English/Spanish fluency, BLS/CPR, and strong customer service. The role emphasizes documentation in EHR and collaboration across departments in a busy setting.

Qualifications

• Bachelor’s degree in Health Education or equivalent perinatal program experience.
• Fluency in English and Spanish may be required per operational need.
• Vaccination and health clearance as specified by policy before start date.
• BLS/CPR certification must be obtained prior to start date and kept current.
• Computer literacy; familiarity with Microsoft Office programs.
• Superior customer service and ability to multi-task.

Responsibilities

• Provide orienting and ongoing CPSP assessments for perinatal members.
• Coordinate care with the care team and refer to clinicians as needed.
• Process perinatal referrals and schedule external appointments.
• Track referrals and ensure member attendance at specialist visits.
• Maintain individualized care plans and resolve access barriers.
• Assist with scheduling, reminders, and documentation in EHR.
• Prepare reports, assist with case conferences, and ensure compliance.
• Support transportation, translation, and other member needs.
