Job summary

Via Care Community Health Center in Los Angeles seeks a CPSP Coordinator to assist in the case management of prenatal patients under the supervision of the CPSP Supervisor. This is a full-time position, working Monday–Friday, with duties spanning documentation, eligibility, and collaboration with clinic

Responsibilities

• Case management of prenatal patients under CPSP supervision.
• Assessments in obstetrics, nutrition, health education and lactation education.
• Program eligibility assessment and referrals for CPSP services.
• Maintain program logs and statistics for internal and external review.
• Coordinate with Los Angeles County Department of Public Health – CPSP.
• Represent the clinic in CPSP matters and support continuous quality improvement.
