Franciscan Health Indianapolis Campus is seeking a Perinatal Advocate to coordinate and implement patient screening for perinatal and postpartum mental health conditions and social determinants of health. The role leads discussions on postpartum experiences, reviews SDoH needs, and provides resources and supportive networks for postpartum mental health and socioeconomic barriers.

• Maintain guidelines for quality assurance programs.
• 1 year Healthcare, Community or Public Health Preferred

_At Franciscan, our Perinatal Advocate is responsible for the coordination and implementation of patient screening of perinatal and postpartum mental health conditions, and social determinants of health (SDoH).

This position provides direction and guidance for the patient by leading discussions on the patient’s postpartum experiences, reviewing SDoH needs, and provide resources, supportive guidance, and supportive networks for postpartum mental health and socioeconomic barriers_.

QUALIFICATIONS
