Department: Homecare – Physical Therapy
Schedule: Days | 8:30 AM – 4:30 PM

Per Visit Physical Therapist Job Summary:

Under the supervision of the HomeCare Rehabilitation Supervisor, the Per Visit Physical Therapist organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.

Per Visit Physical Therapist JobResponsibilities:

• Evaluates and treats patients as prescribed by referring physician. Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device. Educates patient, family and significant others regarding condition, treatment and home program. Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
• Provides clinical supervision of the Physical Therapist Assistant related to the implementation of the patient’s plan of care, in accordance with Medicare Conditions of Participation and state practice regulations. This supervision is specific to patient care and does not include administrative or employment oversight.
• Orients, instructs, and directs other Health Care Team members as needed.

Per Visit Physical Therapist Education and Experience Requirements:

• B.S. Degree and graduate from an accredited school of physical therapy required.
• Connecticut license or Connecticut license eligible required.
• CPR Certification required.
• Minimum of one year experience preferred.
• Valid Connecticut Driver’s License and proof of current insurance coverage required.
