Job summary

Emerald Nursing & Rehab Cozad is seeking a Per Diem Speech-Language Pathologist to join our skilled nursing facility team. You will assess residents, develop individualized treatment plans, and deliver evidence-based therapy to improve communication and swallowing abilities.

You will collaborate with an interdisciplinary team, document progress, and ensure high-quality care while maintaining independence in a supportive facility environment in Nebraska.

Qualifications

• Master's degree in Speech-Language Pathology (CCC-SLP) from an accredited program.
• Current state licensure and CCC-SLP certification.
• Experience working with older adults and individuals with communication and swallowing disorders.
• Ability to work independently and as part of a team.
• Excellent communication and documentation skills.

Responsibilities

• Conduct comprehensive assessments of residents' communication and swallowing abilities.
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
• Provide evidence-based speech-language therapy interventions, including individual and group sessions.
• Monitor and document resident progress, adjusting treatment plans as needed.
• Communicate effectively with residents, families, and healthcare staff to ensure excellent care and support.
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
