Job summary

Park Highlands Nursing & Rehabilitation Center is seeking a Per Diem Speech-Language Pathologist (SLP) to join our in-house team serving residents with person-centered care to achieve positive outcomes.

The SLP will evaluate swallowing or cognitive deficits, develop individualized treatment plans, and provide AAC systems when needed. Flexible schedule and competitive compensation offered.

Qualifications

• SNF/Long-term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate residents’ conditions for swallowing or cognitive deficits.
• Help residents in their goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech and language therapy assessments and develop and implement individualized treatment plans to increase client independence and functioning.
• Provide AAC systems for expressive/language disorders.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures as ordered by physicians.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in residents’ condition.
• Provide accurate and timely billing records and supporting clinical documentation.
