Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin seeks a Per Diem Speech-Language Pathologist (SLP) to provide speech and language services within a skilled nursing and rehabilitation setting. You will work with an interdisciplinary team to develop and implement individualized treatment plans that promote communication, cognition, and swallowing skills.

Qualifications

Speech-Language Pathologist (SLP) designation and licensure requirements as applicable.
Ability to conduct assessments and plan therapy.
Collaborate with interdisciplinary team to implement treatment plans.

Conduct speech and language assessments and screenings to identify patient needs and develop treatment plans.
Provide individual and group therapy sessions for communication, cognitive, and swallowing disorders.
Collaborate with the interdisciplinary team to develop and implement individualized treatment plans.
Document patient progress and maintain accurate records.
Stay current with industry developments and best practices through ongoing education and training.
