Job summary

Rivers Edge Nursing & Rehab in Wisconsin is seeking a dedicated Speech-Language Pathologist (SLP) to assess, diagnose, and treat patients with speech, language, cognitive, and swallowing disorders. You will design individualized treatment plans, lead therapy sessions, and collaborate with an interdisciplinary team to ensure comprehensive patient care.

Responsibilities include maintaining records, communicating with physicians and families, and contributing to quality improvement initiatives.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Completion of clinical fellowship or equivalent experience.
• Current CCC-SLP certification.
• Wisconsin state licensure in Speech-Language Pathology.
• Ability to work effectively in a fast-paced environment.
• Strong communication and interpersonal skills.
• Commitment to providing exceptional patient care and service.

Responsibilities

• Assess and evaluate patients with speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans to address patient goals and needs.
• Provide speech and language therapy services to patients, including individual and group sessions.
• Collaborate with occupational and physical therapists to develop comprehensive treatment plans.
• Maintain accurate and up-to-date records of patient assessments, treatments, and progress.
• Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.
• Participate in quality assurance and quality improvement initiatives to ensure best practices in patient care.
