Job summary

Ryders Health in Connecticut seeks a per-diem Social Worker to support Mystic Healthcare in Mystic, Greentree Manor in Waterford, and Douglas Manor in Windham. You will assist in identifying residents' social and emotional needs and participate in discharge planning and assessments.

Qualifications

• Bachelor's Degree in Social Work or a related Human Services field.
• Ability to make independent decisions when warranted.
• Ability to relate information concerning a resident's condition.

Responsibilities

• Assist in developing policies for the identification of social and emotional needs of residents.
• Participate in community planning related to facility interests and resident/family needs.
• Participate in discharge planning and development of social care plans and resident assessments.
• Interview residents/families to obtain social history.
• Provide information to residents/families about Medicare/Medicaid and other financial assistance programs.
• Consult with staff and community agencies to address resident needs through social worker programs.
• Maintain relationships with medical professionals and other health facilities and organizations.
• Coordinate social service activities with other departments as necessary.
