Job summary

Ryders Health Management is seeking a per-diem Social Worker to support three facilities: Mystic Healthcare in Mystic, CT, Greentree Manor in Waterford, CT, and Douglas Manor in Windham, CT.

The role focuses on identifying residents' social and emotional needs, discharge planning, and resident-centered care in short-term rehab and long-term care settings.

Qualifications

• Bachelor's degree in Social Work or Human Services field required.
• Ability to make independent decisions when warranted.
• Ability to relate information concerning a resident's condition.

Responsibilities

• Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview resident/families to obtain social history.
• Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs.
• Provide consultation to staff and community agencies to solve residents' social needs through program development.
• Maintain a quality working relationship with the medical profession and health facilities.
• Coordinate social service activities with other departments.

Social Worker Responsibilities

• Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident




• Participate in community planning related to the interests of the facility and the services and needs of the resident and family




• Participate in discharge planning, development and implementation of social care plans and resident assessments




• Interview resident/families to obtain social history




• Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs




• Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social worker programs




• Maintain a quality working relationship with the medical profession and other health related facilities and organizations




• Coordinate social service activities with other departments as necessary

Social Worker Requirements

• Bachelors Degree in Social Work or a bachelor's degree in a Human Services field, including but not limited to sociology, special education, rehabilitation counseling, and psychology




• Ability to make independent decisions when circumstances warrant such action




• Ability to relate information concerning a resident's condition
