Qualifications

• Bachelor's degree in Social Work or Human Services field required.
• Ability to make independent decisions when circumstances warrant such action.
• Ability to relate information concerning a resident's condition.

Responsibilities

• Assist in developing and implementing policies for identifying medically related social and emotional needs of residents.
• Participate in community planning related to facility interests and resident and family needs.
• Participate in discharge planning and development of social care plans and resident assessments.
• Interview residents and families to obtain social history.
• Provide information to residents and families about Medicare/Medicaid and other financial assistance programs.
• Consult staff and community agencies to solve residents' social needs through social worker programs.
• Maintain quality relationships with medical professionals and health facilities.
• Coordinate social service activities with other departments as necessary.
