Job summary

Qualifications

Responsibilities

• Develop individualized clinical social work interventions and discharge plans to promote safe transitions of care.
• Participate in interdisciplinary rounds and coordinate with care teams to support patient needs.
• Identify social determinants of health and coordinate referrals to community resources.
