Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our growing team in a SNF setting. As a Physical Therapist, you will evaluate and treat patients following an individualized plan of care while collaborating with facility staff to ensure high-quality outcomes.

You will supervise Physical Therapist Assistants, re-evaluate progress, adjust goals, and maintain timely documentation of evaluations, re-evaluations, discharges, and daily progress notes.

• Delivers high-quality care and communicates progress and outcomes to patients and families.

Responsibilities

• Evaluating new patients and developing plans of care
• Setting measurable functional goals
• Treating patients and educating them on progress and outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress and revising plans of care
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
