Job summary

Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our dynamic team. You will evaluate and treat patients in a SNF setting, following an individualized plan of care and contributing to high-quality outcomes for residents.

You will collaborate with facility staff, supervise PTAs as needed, document progress, and communicate effectively with patients and families to educate on injuries, progress and outcomes.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.

• Providing excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for your patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, educating them on their injury, progress and probable outcomes.
• Supervising Physical Therapist Assistants.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
