The New Jewish Home's Certified Home Health Agency (CHHA) is seeking dedicated Per Diem Speech-Language Pathologists to provide home-based services across Manhattan. You will conduct comprehensive assessments, develop individualized plans, and deliver interventions to maximize communication, swallowing, safety, and quality of life.

Qualifications

• Current, unrestricted New York State license as a Speech-Language Pathologist.
• Experience working with a geriatric population.
• Availability to provide home visits throughout the Manhattan service area.
• Strong assessment, clinical reasoning, communication, and documentation skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.

Responsabilités

• Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician.
• Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders.
• Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals.
• Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations.
• Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care.
• Participate in case conferences, team meetings, and care planning discussions.
• Maintain required departmental and regulatory records in a timely and accurate manner.
• Assist in maintaining therapy equipment, supplies, and work areas.
• Participate in professional development and educational activities to remain current with best practices and advancements in the field.
• Perform other duties as assigned.
