Job summary

The New Jewish Home's CHHA is seeking dedicated Per Diem Speech-Language Pathologists to join our Community Services team in Manhattan. You will provide home-based speech therapy to adult and geriatric patients, conduct assessments, develop individualized treatment plans, and deliver interventions to maximize communication, swallowing function, safety, and quality of life.

Collaboration with an interdisciplinary team is essential.

Responsibilities

• Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician.
• Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders.
• Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals.
• Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations.
• Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care.
• Participate in case conferences, team meetings, and care planning discussions.
• Maintain required departmental and regulatory records in a timely and accurate manner.
• Assist in maintaining therapy equipment, supplies, and work areas.
• Participate in professional development and educational activities to remain current with best practices and advancements in the field.
• Perform other duties as assigned.
