Job summary

BAYADA Home Health Care at Bayhealth is seeking an experienced Speech Language Pathologist (SLP) for a per diem opportunity, performing home health visits in Dover, DE. You will be part of a multidisciplinary team delivering rehabilitative care to adults and geriatric patients in Kent County.

Requirements include a current DE license or eligibility for licensure, ASHA CCC-SLP or eligibility, and EMR experience.

Qualifications

• Current DE Speech Language Pathologist license.
• ASHA CCC-SLP certification or eligible.
• Equivalent education/work experience allowing certification.
• Must be able to obtain state licensure if not currently licensed.
• Proficient with electronic medical records (EMR).

Responsibilities

• Make home visits to clients in designated Dover/Kent County area.
• Perform evaluations to diagnose and treat speech, language, swallowing difficulties.
• Develop and implement individualized treatment plans.
• Continuously assess and revise the care plan with the care team.
• Educate patients and family members on rehabilitation and exercises.
• Document observations and interventions using a touch pad tablet.
