BAYADA Home Health Care at Bayhealth is seeking an experienced Speech Language Pathologist (SLP) for a full-time position delivering home health visits in the Dover, DE area. The role involves patient evaluations, treatment planning, and collaboration with a multidisciplinary team to support in-home rehabilitation and independence.

Responsibilities include making home visits, developing and adjusting plans, educating patients and families, and documenting care in an EMR system.

Qualifications

• Current DE license as a Speech Language Pathologist.
• ASHA CCC-SLP or equivalent for certification process.
• Experience or education path toward ASHA certification.
• Experience with EMR systems preferred.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform evaluations and provide therapeutic interventions for speech, language, swallowing difficulties.
• Develop and revise treatment plans based on evaluations and goals.
• Educate patients and families on rehabilitative care.
• Document observations and interventions in EMR on a touch pad tablet.
• Collaborate with multidisciplinary care team.
