Responsibilities

• Assumes responsibility for a group of residents including medication/treatment administration.

• Performs ongoing assessments to develop an interdisciplinary care plan.
• Communicates effectively with residents, families, and the care team.
• Documents information accurately and timely as per policy.
• Supervises CNAs & LPNs to ensure adherence to the plan of care.
• Participates in ongoing education, inservices and quality improvement activities.
