Job summary

Pathways Healthcare is a leading home health and hospice provider delivering patient-centered care in the community. This per diem RN/LPN role serves Arlington and surrounding areas, requiring strong clinical skills and a compassionate bedside manner.

The ideal candidate holds MA RN or LPN licensure, with 2+ years in geriatric care, and a current BLS CPR certification. A valid driver’s license and reliable vehicle are required to travel to patient homes.

Qualifications

• Active RN or LPN license in MA.
• 2+ years nursing experience, geriatrics preferred.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Conduct initial patient/family assessments to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care.
• Collaborate with physicians, NPs, and teams to create plans of care.
• Address inquiries from patients, caregivers, and medical teams.

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active RN or LPN license in the state where the position is based. LPNs are encouraged to apply.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
