Job summary

IntelliResume in Independence, OH, seeks an RN Homecare PRN to provide direct nursing care under established policies, coordinating care and supervising support staff in a home care setting.

The role involves admissions assessments, safety evaluations, pain management, and close collaboration with patients, families, and healthcare teams. Ohio RN licensure and BLS are required; BSN preferred.

Qualifications

• Graduate of an accredited school of professional nursing; BSN preferred.
• Minimum one year clinical RN experience in an acute medical-surgical setting; infusion therapy experience preferred.
• Minimum of six months experience as a home care nurse for home care only.
• Current state RN licensure.
• Basic Life Support (BLS) certification through American Heart Association.
• Valid Ohio driver’s license upon hire; per policy 8‑175 thereafter.
• Proof of automobile insurance coverage, upon hire; per policy 8‑175 thereafter.

Responsibilities

• Provide direct nursing care per policies, procedures, and protocols.
• Perform admissions/reassessments within specified time frame.
• Assess safety risks, abuse, and neglect.
• Coordinate care as primary liaison for disciplines.
• Assess and document pain; manage patient pain.
• Prioritize problems with patient, family, and healthcare team.
• Maintain up-to-date documentation of patient condition and care effectiveness.
• Escalate changes in patient condition to physician as appropriate.
• Supervise LPNs, home care aides, and family/caregivers.
• Perform other duties as assigned.
